The challenge of sustaining physical activity in middle-aged and older adults requires moving beyond initial enrolment in community sport. This article aims to develop a conceptual model integrating pedagogical support, digital monitoring and organizational conditions for personalized participation. A targeted narrative review of 16 sources was conducted, combining five publications addressing the Kazakhstan context and sensor-based gait assessment with international research and guidelines. Study designs and findings were compared, barriers were grouped thematically, and a conceptual model was constructed. Four interconnected components were identified: program accessibility, individualized exercise, adherence support and functional monitoring. A participant support pathway and a matrix linking barriers to professional actions and measurable outcomes are proposed. Classification based on previous falls is distinguished from prospective risk prediction, while attendance is separated from physical activity volume and quality-of-life outcomes. The model provides a framework for coordinating instructors, program managers and monitoring specialists. Its effectiveness remains to be tested prospectively, with separate assessment of activity, participant retention, functional change and safety. Digital tools are positioned as aids to professional decision-making rather than autonomous exercise prescription systems
